How Cheek Fillers Work

The midface — the zone from the cheekbones down to the nasolabial folds — contains multiple anatomical fat compartments that lose volume progressively with age. As these compartments deflate, the overlying skin and soft tissue descend, creating a flattened cheek profile, deepened smile lines, and a general shift from the triangular, upward-pointing facial shape of youth to a more rectangular, downward-pointing shape.

Cheek filler works by physically replacing that lost volume in the deep anatomical compartments where it belongs. Juvéderm® Voluma® XC is a hyaluronic acid (HA) gel formulated using Allergan's proprietary Vycross cross-linking technology, which creates a cohesive, high-lift product engineered specifically for deep midface injection. When placed in the deep fat compartments of the malar and submalar midface, it provides structural lift and volume support that raises and reshapes the overlying tissue.

Hyaluronic acid is a naturally occurring polysaccharide found throughout the body — particularly in skin and connective tissue — where it plays a central role in hydration and structural support. The cross-linking process in Voluma XC extends how long the HA gel resists enzymatic breakdown, supporting results that clinical studies show can last up to 24 months in many patients.

Safety & Natural Results

  • Conservative starting volume. We begin with the minimum volume needed to achieve the correction goal, assess the result, and offer additional volume only if the patient and clinician agree it is warranted. We do not default to a preset syringe count.
  • Deep-plane placement. Voluma XC is designed for deep injection into the malar and submalar fat compartments. Superficial placement in the wrong tissue plane creates the shelf-like or artificial appearance associated with "bad cheek fillers." Correct depth placement integrates with anatomy rather than sitting on top of it.
  • Anatomy-matching, not symmetry-forcing. Natural faces are not perfectly symmetrical. Chasing perfect symmetry by adding excessive volume to the dominant side creates an overfilled appearance. We work within the patient's natural anatomical range.
  • Cannula technique option. A blunt-tip cannula — rather than a sharp needle — allows filler to be distributed smoothly through natural tissue planes, reducing bruising risk and enabling precise placement without repeated puncture.

Pre-Treatment Preparation

In the days before your appointment, avoid blood-thinning medications (aspirin, ibuprofen, fish oil, vitamin E) if medically appropriate. Avoid alcohol for 24 hours prior. Arrive with clean skin, no active cold sore or skin infection in the treatment area, and well-hydrated. If you have a history of cold sores, discuss antiviral prophylaxis with your clinician at consultation.

Treatment Appointment

Topical numbing cream is applied to the midface and allowed to take effect for approximately 20 minutes. Your licensed RN clinician then carefully places Voluma XC into the deep compartments of the midface using a fine needle or blunt-tip cannula depending on the injection site and treatment plan. The filler contains lidocaine, which provides additional comfort during placement. The full treatment appointment typically takes 30–45 minutes. Results are visible immediately as filler is placed.

Immediate Aftercare

Mild swelling, tenderness, and possible bruising are expected and normal in the hours and days following treatment. Apply a cold compress gently to the treated area to minimize swelling. Avoid touching, pressing, or massaging the injection sites for 24 hours. Avoid strenuous exercise, alcohol, excessive heat (saunas, hot yoga, steam rooms), and lying face-down for 24–48 hours.

Two-Week Follow-Up Assessment

Your final settled result emerges at approximately two weeks. If you have any concerns about your result — insufficient correction, asymmetry, or any unexpected changes — contact us during this window. A follow-up assessment is available to evaluate your outcome and determine whether any additional correction is warranted.

Aftercare

The First 48 Hours

  • Mild swelling is normal and expected — it does not represent your final result. Apply a cool (not icy) compress gently to the cheeks for short intervals in the first few hours. Avoid touching, massaging, or pressing the injection sites. Sleep on your back if possible to avoid direct pressure on the treated area. Avoid strenuous exercise, alcohol, and heat exposure (hot tubs, saunas, heated yoga) for the first 24–48 hours. Bruising, if it occurs, can be covered with concealer after 24 hours.

Days 3–14

  • Residual swelling continues to resolve through the first two weeks. Avoid facial massage or laser/energy treatments over the midface during this period. You may return to normal skincare and light makeup immediately. Protect the treated area with SPF 30+ sunscreen. Avoid intense sun exposure or tanning for two weeks. At approximately 10–14 days, your settled result is visible — this is the result to evaluate, not the day-one appearance.
